The Case Manager will collaborate with clients with severe mental illness to develop and implement effective treatment plans while ensuring their basic needs are met. They will also participate in a 24/7 on-call crisis intervention system and maintain accurate clinical documentation.
Candidates must hold a bachelor's degree in social work, psychology, or a related human service field. At least one year of experience working with individuals with severe and persistent mental illness is required, along with a valid driver's license and reliable transportation.

We support community members who are at most risk for long-term psychiatric hospitalizations. This multidisciplinary team provides a wrap-around approach to support adults with severe and persistent mental illness. We meet clients in their homes and neighborhoods, with the goal of helping them to maintain stability and be successful within the community.
