The Case Manager provides central support within an interdisciplinary team by conducting behavioral health assessments and service planning for SMI adults. They are responsible for facilitating groups, managing intake screenings, and identifying gaps in care to ensure effective coordination.
Candidates must hold a bachelor’s degree in a behavioral health field with at least one year of relevant experience, or a combination of education and experience totaling two years. Additionally, applicants must be at least 21 years old, possess a valid driver’s license, and obtain fingerprint clearance.

The Case Manager serves as a central support within the interdisciplinary team, working primarily with SMI adults. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.
Starts $17.50 hourly
