The Mental Health Practitioner will provide treatment, service coordination, and crisis intervention for clients at the FACT north office. They are responsible for developing recovery plans, providing vocational and supportive counseling, and maintaining accurate service documentation.
Candidates must hold a Master’s degree in a behavioral science, Social Work, or Counseling. Experience working with adults with severe and persistent mental illnesses is required, along with a valid Florida Driver's License and the ability to pass a Level 2 background check.
We are seeking a caring and compassionate Mental Health Practitioner to provide treatment and service coordination for clients at our FACT north office. They will be responsible to carry out rehabilitation and support functions and assist in treatment; substance abuse services; education, support, and consultation to families; and crisis intervention under the clinical supervision of staff with professional degrees.
assume primary responsibility for developing, writing, implementing, evaluating, and revising overall recovery goals and plans in conjunction with the ITT (Individual treatment team) Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put members at risk Provides individual vocational-supportive counseling to enable members to identify vocational strengths and problems, establish vocational or career goals and plans to reach them, and recognize and target symptoms of mental illness that interfere with work Provides individual supportive therapy (e.g., problem solving, role-playing, modeling and support), social-skill development, and assertiveness training to increase member social and interpersonal activities in community settings. Prepares written documentation for each service conducted. Performs other duties as requested.
Master’s degree in a behavioral science, Social Work , or Counseling Experience with adults with severe and persistent mental illnesses or with individuals with similar human-services needs.
