The Physical Therapist will provide physician-prescribed therapy to patients in their homes to improve physical disabilities and promote independence. They are responsible for conducting initial assessments, developing care plans, and performing reassessments every 30 days.
Candidates must hold a degree in physical therapy from an accredited program and possess a current state license. Two years of experience in a home health setting and familiarity with Medicare OASIS documentation are preferred.

A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.
