The nurse is responsible for the intake and maintenance of psychiatric clinic clients, providing triage and medical assessments. They also coordinate care with community providers and administer medications as prescribed.
Candidates must hold an active LPN or RN license in the state of Oregon and have at least 2 years of professional nursing experience. A valid driver's license and personal auto insurance are also required.

Under the direction of a Licensed Medical Professional, the Nurse is responsible for working with the Psychiatric Providers and the Clinical Staff to assist in the intake and maintenance of Psychiatric Clinic clients. The Nurse will also provide support functions to the providers.

5. Coordinate with community providers to obtain needed medical supports such as care-givers, durable equipment purchases and maintenance.
6. Coordinate and consult with PCP, medical specialists, hospitals, rehabilitation programs and other services on behalf of mutual patients.
7. Provide in-home skills training to patients to monitor and effectively manage medical conditions such as diabetes, COPD and Hepatitis.
8. Administers medications and injections as prescribed by the LMP. 9. Screens participants for medication side effects and medical conditions. 10. Provide individual and group skills training regarding nutrition, medications, and physical wellness as components of mental health recovery.
11. Participate in trainings and meetings. 12. Provide education and training to team members regarding medications and potential side effects.
13. Maintain the medication room to proper OAR standards and administration best practices. 14. Complete required documentation within the specified timelines stated below under “Documentation Requirements.”

To perform the job successfully and to meet the standards of professionalism, this individual should demonstrate the following competencies:
This individual will be licensed as an LPN or RN in the state of Oregon. At least 2 years’ experience in professional nursing field work is required. Experience working with adults is preferred.
Valid Oregon driver’s license, clean driving record. Personal automobile insurance is required if clients are transported or any agency business is conducted by the employee using his/her personal vehicle. Must possess an active RN or LPN Nursing license in the State of Oregon.
All clinical documentation of services provided to clients or on behalf of the client must be completed within 48 hours of the service completion. For services provided at the end of a month, all services must be documented by the second (2nd) business day of the following month. This is to ensure that all month end reporting is completed within a timely manner.

Driving is a requirement for this position. Proof of insurance must be provided to the HR department. Coverage must remain current.
The majority of the work is performed indoors at the office, in the client’s home or community setting, however, the employee is also exposed to outside weather conditions; extreme cold and extreme heat. The noise level in the work environment is usually moderate. Employees may be exposed to second hand smoke, dirty working environment, agitated clients, and possibly aggressive pets when working with clients in their home.
