Coordinate services for youth in the Comprehensive Community Services program to ensure mental health and well-being needs are met. This includes performing clinical assessments, developing treatment plans, and facilitating collaboration between service teams and families.
Requires a Bachelor's degree in social work or a related field, with a Master's degree and professional licensure preferred. Candidates should have a valid driver's license, reliable transportation, and preferably two years of experience working with children and families.

Our next Service Facilitator- Comprehensive Community Services will be responsible for the coordination of services for youth in the Comprehensive Community services program of Washington County and to ensure the needs of youth and family are met by providers to facilitate greater overall mental health and well-being.
