• Compensation & Benefits Base Salary: $140,000 – $190,000 (based on experience) Performance Bonus: Up to 25% of base salary, tied to quality metrics Health Coverage: Medical, dental, vision, life, and pet insurance Retirement: 401(k) with employer match Additional: PTO, mileage reimbursement or company vehicle, flexible scheduling What You'll Do Conduct PT evaluations and reassessments independently Develop and manage individualized plans of care Deliver skilled therapy targeting mobility, function, fall prevention, and independence Collaborate with PTAs on an occasional, well-supported basis Communicate with physicians and the interdisciplinary team Document with AI-assisted charting support Serve as the primary PT voice in patient care planning

Requirements

Active California Physical Therapist license Valid California driver's license Strong clinical judgment and time management Home health experience preferred but strong outpatient, inpatient, or rehab PTs are encouraged to apply Ready to Connect?
