Greater Mental Health of New York’s Care Management Program helps individuals access essential services, such as medical specialists, transitioning from hospital admissions, and securing social supports, such as housing. The Care Manager is responsible for delivering comprehensive care management services to individuals qualifying for the Health Homes Care Management program in Rockland County. This includes assessing, overseeing, and coordinating various needs such as medical, behavioral health, substance abuse, and social services. The role involves direct client care in the community or via telephonic services, participation in meetings, and adherence to agency policies while supporting recovery and utilizing evidence-based practices.
• Hours per week, reporting structure, and location are subject to change per program needs.
This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter. You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
