The RN is responsible for providing patient-centered hospice care, conducting assessments, and managing care plans for patients and families. They provide coverage across homecare teams and coordinate with an interdisciplinary team to ensure high-quality end-of-life care.
Candidates must hold a current, unrestricted Registered Nursing License and possess a valid driver's license with a reliable vehicle. One year of nursing experience and proficiency in electronic health records are preferred.
