The Physical Therapist is responsible for assessing the needs of referred patients and creating treatment plans within a home care setting. They provide therapy services while collaborating closely with physicians and case managers.
Candidates must hold a degree in Physical Therapy and a current, unencumbered license to practice in the state of employment. A minimum of two years of experience as a Physical Therapist and a valid CPR certification are required.
The Physical Therapist (PT) is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment. The PT provides therapy services defined in treatment plans while working cooperatively with physicians and case managers. The PT performs a variety of tasks and is encouraged to use a certain degree of creativity and latitude. COVERAGE AREA Blanco, Johnson City and surrounding areas
Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation Current and unencumbered licensed to practice as a Physical Therapist within the state of employment. Two (2) years of appropriate experience as a Physical Therapist. Community/home health experience is preferred. Demonstrates good verbal and written communication and organization Possess and maintain valid CPR Must have obtained in-person with a practical skills evaluation as part of the final test. Access to reliable transportation.
