Conduct outreach and build relationships with unhoused individuals to connect them with essential care and support services. Monitor participant stability in permanent housing and administer mental health and addiction assessments.
Preferred candidates hold a Bachelor's degree in a related field or are a Licensed Practical Nurse, though case management experience may be substituted. Experience with homeless populations and behavioral health services is highly desired.

Make referrals to necessary care and support services utilizing warm handoffs. Work closely with any existing care team(s) of individuals being served. Monitor engagement in services and status of permanent housing through continued connection to ensure stability, independence and wellbeing in their new community. Administer assessments related to mental health, addiction, and/or social determinants of health (training is provided). Crisis management and safety planning using the Columbia Protocol and other best practices. Completes all required documentation, in timelines in accordance with program standards. Care Manager Qualifications: Preferred candidates for the Care Manager position would have a Bachelor’s degree or higher, preferable in psychology, social work, sociology, or related field or be a New York State Licensed Practical Nurse (LPN). Past case management work experience will be considered in lieu of Bachelor’s degree. Experience providing advocacy with homeless and/or precariously housed populations preferred but not required. Experience as provider and/or recipient of behavioral health, substance use and/or homeless services. Knowledge of: homeless resources within the community; treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff; community-based organizations and opportunities for continuing wellness development of participants; Intentional Peer Support models, best practices and methods of implementation. Awareness of the various personal and systemic issues that hinder access to care. Bi-lingual in Spanish and English preferred. Care Manager Required Skills and Abilities: Candidates are required to possess the ability to understand, support and operate from a strength-based approach, display professionalism in all settings, and recognize the importance and benefits of relationship building and collaborating with other service providers. Candidates must have the ability to solve complex problems using creative solutions and act consistently in a manner that demonstrates dignity and respect for participants is required, as well as the ability to de-escalate participants in times of crisis. Candidates must have respect for and high level of comfort around people experiencing psychiatric crises and the ability to utilize critical thinking, problem-solving and de-escalation skills to assist with creating and maintaining an environment that promotes recovery.
