Responsibilities

• Conduct PT evaluations and develop individualized treatment plans based on assessment findings
• Implement therapeutic exercises, gait and balance training, functional mobility training, and task-specific activities
• Administer modalities and assistive devices as prescribed; monitor patient response and adjust plans of care
• Document progress notes, treatment plans, and outcomes in patient records; communicate with supervising PT and interdisciplinary team
• Ensure patient and staff safety during transfers, mobility activities, and therapeutic interventions
• Provide patient and caregiver education on rehabilitation goals, home programs, and safety strategies; participate in discharge planning as appropriate

Requirements

• Active license to practice Physical Therapy in California or eligibility for CA licensure
• Doctor of Physical Therapy (DPT) degree or equivalent
• Experience in physical therapy clinical settings; SNF or home health experience preferred
• Ability to work in a skilled nursing facility environment and participate in a travel assignment
• Strong documentation, communication, and collaboration skills; able to work independently and as part of a team
• Ability to meet credentialing and background-check requirements typical for travel assignments
