Responsibilities

• Conduct comprehensive PT evaluations for home health patients and develop an evidence-based Plan of Care aligned with HCHB and OASIS requirements
• Deliver individualized therapy focused on gait, balance, strength, mobility, pain management, and functional rehabilitation
• Provide patient and caregiver education, training, and home program instruction to promote independence and safety
• Document assessment findings, treatment plans, progress notes, and OASIS data accurately in the medical record; communicate changes with the interdisciplinary team
• Ensure patient safety during visits, maintain infection control standards, and adhere to AHA BLS requirements
• Coordinate with care teams to optimize discharge planning and transitions of care

Requirements

• Doctor of Physical Therapy (DPT) degree
• California Physical Therapist license (or eligible for licensure)
• AHA BLS certification
• Minimum 1 year of home health experience
• Proficiency with HCHB and OASIS documentation
• Ability to perform home health PT visits, travel between patient locations, and work the listed Monday–Friday daytime schedule
