Coordinate access to treatment, recovery supports, and community resources for individuals receiving substance use treatment. Develop individualized service plans and monitor client progress using trauma-informed and person-centered approaches.
Requires a high school diploma and at least one year of experience with mental health or substance use disorders. Must obtain CDC-A certification within 90 days and possess a valid driver's license for community-based work.
Utizlizes a trauma-informed approach to client care. Conducts initial and ongoing needs assessments and assist clients in identifying recovery goals. Coordinates and links clients to appropriate treatment services, primary care, housing, employment, legal, social and recovery supports. Monitor client progress towards treatment plan goals. Utilizes motivational enhancement strategies to assist clients in achieving personal recovery goals. Advocate on behalf of clients and support engagement in recovery-oriented and person-centered services. Ensure accurate and timely documentation of all services in compliance with agency policy and regulatory standards. Participates in case conferences, team meetings and trainings as required. Aids in the development of individualized service plans in collaboration with the client and the interdisciplinary team. Maintains strong clinical and ethical boundaries with clients. Facilitates group counseling as required for training and development. Qualifications High school diploma or equivalent. Associates or Bachelor’s degree in Psychology, Counseling, or a related Human Service Field preferred. CDC-A certification required within 90 days of hire. Valid driver's license and insurable driving record. Position requires community based work. Must be willing to transport clients. Minimum of one year of experience working with individuals with mental health or substance use disorders. Strong interpersonal and communication skills. Ability to work independently and as part of a multidisciplinary team. Experience with writing clinical documentation. Familiarity with trauma-informed care, harm reduction and motivational interviewing techniques. Strong knowledge of community resources and systems. Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams. Adept at learning and navigating electronic health record systems.
