  • Coordinates with other disciplines when indicated.
  • Prioritizes care needs and sets measurable goals and time frames.
  • Includes patient and family in the planning.
  • IMPLEMENTATION: Implements interventions based on nursing and medical plan of care.
  • Manages patient population, diagnosis and age specific needs through appropriate adaptation of interventions.
  • Appropriately utilizes equipment and supplies (including following established policies, procedures and protocols governing the transport, handling and storage of intravenous fluids).
  • EVALUATION: After implementation, reassesses the patient to verify effectiveness of interventions.
  • Updates the plan of care to reflect current care needs.
