The LPN provides comprehensive oversight and hands-on nursing care to program participants, including medication administration and health assessments. They are responsible for maintaining accurate medical records and collaborating with interdisciplinary teams to ensure optimal health outcomes.
Candidates must hold a current PA RN/LPN license, CPR/First Aid certification, and a valid driver's license. Required clearances include Act 33/34 and FBI, along with completion of a physical, TB test, and Hep B titer.
The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.
