Coordinate behavioral health, medical, and social services to help clients overcome barriers and achieve recovery goals. Develop individualized service plans and collaborate with community partners and multidisciplinary teams to ensure person-centered care.
Requires a Bachelor's or Master's degree in human services, a license as a Psychologist, or a Registered Nurse license. Preferred experience includes behavioral health, substance use treatment, or case management.
The QLA Case Manager/Care Coordinator Level 2 assists individuals in accessing essential behavioral health, medical, social, educational, and community resources. This position provides care coordination and support services to help clients overcome barriers, improve stability, and achieve recovery goals.
The successful candidate will work collaboratively with community partners, healthcare providers, recovery organizations, and internal treatment teams to ensure individuals receive comprehensive, person-centered care.
