Hygea Healthcare is seeking a versatile and clinically strong dual‑certified FNP/PMHNP to support integrated medical and psychiatric services across our addiction and behavioral health programs. This role is ideal for providers who enjoy whole‑person care and collaborative practice. Key Responsibilities Perform psychiatric evaluations and medication management Provide medical assessments, chronic disease management, and urgent clinical support as needed Complete H&Ps, medication reconciliation, and ongoing medical oversight Collaborate with nursing, UR, billing, and leadership to ensure compliance and medical necessity Participate in interdisciplinary rounds and quality improvement initiatives Maintain accurate, timely documentation in Kipu

SUD Counselor- Job Summary The Substance Use Disorder Counselor’s role is to support others in recovery from a substance use disorder. The SUD Counselor will provide individual, group, and family counseling sessions to assist recovering individuals in preventing relapse and promoting long-term recovery. The SUD counselor will use an outcomes-based approach and evidenced-based practices to achieve results. Essential Job Functions Conduct confidential, quality assessments, that are gender-responsive and trauma-informed, utilizing ASAM criteria to determine the appropriate level of care. Use a strengths-based and trauma-informed approach to work collaboratively with clients and internal and external Hygea Healthcare treatment team members to help support clients in their treatment and recovery. Provide clinical consultation to referral resources, mentors, family members, and others to help. Conduct individual, group, and family counseling sessions. Assist clients to meet all requirements of their care and maintain an update and individualized treatment plan for each client. Use evidence-based practices to meet the clinical needs of each client. Evaluate the effectiveness of the treatment program and communicate needs, progress, and barriers. Participate in agency and community committees. Provide education by presentation or workshops to clients, families, or the community at large. Consistently follow agency requirements for documentation completion to insure timely filing for billing and regulatory compliance. Availability to be on-call for patient care and required holiday ( One per year ).
