Provide routine telehealth visits and manage personalized care plans for Medicare patients. Collaborate with an integrated care team to ensure high-quality preventive care and thorough EHR documentation.
Must be an MD or DO, Board Certified or Eligible in Family Medicine, Internal Medicine, or Geriatrics. Requires an active medical license and credentialing with major plans like UHC, Aetna, and Humana.
What We’re Looking For MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics Active medical license — multi-state licensure a plus Credentialed with major plans: UHC, Aetna, Humana Strong interest in preventive care, senior health, or chronic condition management Tech-savvy and comfortable practicing via telehealth Collaborative, mission-driven mindset with excellent communication skills
