• Functions: Ability to administer skilled nursing care for clients of all ages in their place of residence under the direction of a Registered Nurse. Coordinate care with the interdisciplinary team, patient/family and referring agency. Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care. Provides patient and caregiver education.
