The RN plans and directs home care services, performing initial and ongoing physical assessments to develop and revise patient care plans. They coordinate care with physicians and community resources while managing a caseload as a Case Manager.
Requires a nursing degree (Associate or Bachelor's), a valid RN license, CPR certification, and at least one year of RN experience. Candidates must have a valid driver's license, an insured vehicle, and the physical ability to transfer patients.
Graduate from an accredited school of nursing, associate or bachelor’s degree. Valid/active nursing license. 1 year of previous experience as a RN required Must be a licensed driver with an automobile in good working order and insured in accordance with the organization's requirements. Valid CPR certification is required. Recent TB test, required. Prior home health care or acute care experience preferred. Management experience preferred but not required. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending, kneeling or crouching. Must be able to lift, position or transfer patients and lift supplies and equipment. Must have visual acuity and hearing to perform required nursing skills. Why Summit?
