The Case Manager provides non-clinical support to Soldiers to ensure medical readiness and deployability. Responsibilities include managing cases, coordinating follow-up care, and supporting medical readiness events.
Candidates must be an LPN, Licensed Social Worker, or hold a Bachelor's degree in a health-related field. A minimum of one year of case management experience is required.
The Case Manager provides non-clinical case management support for Soldiers requiring medical readiness assistance. This position ensures accurate case tracking, documentation, and communication to support deployability and readiness objectives. Work Schedule Full-Time 40 Hours per Week Monday through Friday 7:30 AM – 4:30 PM Eastern Time On-site support required Travel throughout Ohio may be required for SRP and Medical Readiness Events Assigned support for approximately 9–15 readiness events annually depending upon brigade assignment
Manage assigned Soldier medical readiness cases. Coordinate follow-up care and appointments. Maintain records within eCase, MEDPROS, HRR, and MEDCHART. Process Release of Information (ROI) requests. Conduct monthly case reviews. Communicate with Soldiers, Units, MRNCOs, and medical personnel. Support SRP and Medical Readiness Events. Track and report medical readiness metrics.
Licensed Practical Nurse (LPN), Licensed Social Worker, or Bachelor's Degree in a health-related field. Minimum one (1) year of case management experience. Strong communication and organizational skills. Proficiency with Microsoft Office applications.
