The Physical Therapist is responsible for completing patient assessments upon admission and establishing individualized treatment plans. They also educate patients and their caregivers to ensure the successful implementation of therapy goals.
Candidates must be licensed Physical Therapists in Oregon with a degree from an accredited program and a valid CPR certification. A National Provider Identifier (NPI) is required upon hire, and home health experience is preferred.
