Are you passionate about enhancing lives? We are seeking a Physical Therapist to join our team at Mapleton Community Home in Mapleton, MN.

• As a Physical Therapist, you will be responsible for directly participating in physical therapy treatment interventions and services, in accordance with state practice guidelines.
• This position provides direct patient care to primarily adult and geriatric patients.
• Essential Key Job Responsibilities: Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to patient’s prior level of function.
• This includes but is not limited to the following areas: gait and balance, mobility, range of motion, strength, pain, conducting home assessments, discharge planning, development of maintenance programs and other related patient care issues.
• Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice.
• Monitor patients' progress and adjust treatment plans as needed.
• Provides appropriate supervision for physical therapist assistants in compliance with licensure, regulatory and company requirements.
• Maintain accurate and up-to-date patient records.
• Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff.
• Participate in continuous quality improvement activities.

Requirements

Education and Experience: Successful completion of an accredited Physical Therapy program. Knowledge, Skills, and Abilities: Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong problem-solving and critical thinking skills. Compassionate and patient-centered approach to care. License and Certification: Current valid Physical Therapist license to practice in appropriate state.
