We're building our PRN Palliative Care LPN team and offering competitive per-visit pay with a flexible schedule.
Pay Rates – Per Completed Visit Per-Visit - $42 Downtime (pre-approved wait time) - $30/hr "Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.
Service Area & Schedule Cleveland / Northeast Ohio | Weekdays, weekends, evenings, on-demand - with a flexible schedule.

Requirements
Must-Have: Active LPN license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical nursing experience Preferred / Nice-to-Have: Prior palliative care, hospice, or home health experience Experience supporting patients and families through serious illness Familiarity with HCHB or similar home health EMR Strong communication and family support skills
