$45 - $115 / HOUR
Provide physical therapy services to patients in their homes, including start-of-care, evaluations, and revisits. Complete all clinical documentation and order uploads within 24 hours of the visit.
Requires an active Ohio PT license, BLS certification, a valid driver's license, and at least one year of clinical experience. OASIS certification and prior home health experience are preferred.
Must-Have: Active PT license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical physical therapy experience Preferred / Nice-to-Have: OASIS certification or demonstrated OASIS experience Prior home health experience Familiarity with HCHB or similar home health EMR
