Provide direct patient care and symptom monitoring for hospice patients in their homes according to established care plans. Collaborate with the interdisciplinary team to provide education and emotional support to patients and families during end-of-life care.
Requires a valid LPN license and BLS certification, with a strong preference for experience in hospice or palliative care. Candidates should be familiar with EMR systems and possess knowledge of geriatrics, dementia, and pain management.

We're looking for a compassionate, skilled LPN to join our Hospice team in a full-time, field-based role serving patients and families throughout the Cleveland / Northeast Ohio area. Valid LPN license in good standing with relevant state licensing boards. Proven experience in hospice or palliative care settings is highly preferred. Familiarity with EMR/EHR systems such as HCHB Knowledge of medical terminology related to geriatrics, dementia care (including Alzheimer's), infectious disease care, and pain management. Experience working with individuals with developmental disabilities or in assisted living environments is a plus. Strong assessment skills combined with physiology knowledge to evaluate patient needs effectively. Excellent communication skills for triage situations and family education; basic life support (BLS) certification is necessary.
