Hospice LPN

We're looking for a compassionate, skilled LPN to join our Hospice team in a full-time, field-based role serving patients and families throughout the Cleveland / Northeast Ohio area. Compensation $30.00-45.00 per hour Expected 40 hours per week Additional coverage opportunities available in Home Health and Palliative Care Schedule & Location Field-based | Cleveland / Northeast Ohio | Full-Time, 40 hrs/week What Sets VNA Apart AI-assisted documentation - spend your time with patients, not paperwork 4-star CMS agency with CHAP accreditation and a 124-year legacy in Northeast Ohio Mileage reimbursement at the IRS standard rate ($0.70/mile) Paid training & CEUs through Relias Limited weekend/holiday requirements - we respect your time off Responsive support team - scheduling, intake, clinical supervisors, triage, and after-hours coverage so you're never on an island Public Service Loan Forgiveness (PSLF) eligible employer - qualifying employees may be eligible for federal student loan forgiveness after 120 payments One of the only home health agencies in Northeast Ohio with a dedicated Behavioral Health nursing program What You'll Do Provide direct patient care to hospice patients in their homes in accordance with the established plan of care Monitor and document patient symptoms, comfort levels, and changes in condition Administer medications and treatments as directed by the RN Case Manager or physician Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals Provide education and emotional support to patients and families navigating end-of-life care Maintain accurate and timely documentation in the EMR within required timeframes Provide coverage in Home Health and Palliative Care settings as needed

Requirements

Valid LPN license in good standing with relevant state licensing boards. Proven experience in hospice or palliative care settings is highly preferred. Familiarity with EMR/EHR systems such as HCHB Knowledge of medical terminology related to geriatrics, dementia care (including Alzheimer's), infectious disease care, and pain management. Experience working with individuals with developmental disabilities or in assisted living environments is a plus. Strong assessment skills combined with physiology knowledge to evaluate patient needs effectively. Excellent communication skills for triage situations and family education; basic life support (BLS) certification is necessary.
