Case Manager Neulife Rehabilitation is currently seeking a skilled and compassionate Case Manager to join our team in Mt Dora, Florida. As a Case Manager, you will play a vital role in providing support and guidance to individuals in need of rehabilitation services.

Responsibilities

Conduct assessments and evaluations to determine individual needs Develop and implement individualized treatment plans Coordinate and monitor the delivery of services to clients Collaborate with healthcare professionals and service providers Advocate for clients' needs and rights Maintain accurate and up-to-date documentation

Requirements

Bachelor's degree in social work, psychology, or related field Previous experience in case management or a related role Strong interpersonal and communication skills Knowledge of community resources and support services Ability to multitask and work in a fast-paced environment
