The role involves assessing and engaging service users with complex needs, specifically those with histories of rough sleeping and substance misuse. Responsibilities include providing screening, recovery planning, and supporting social reintegration to help users lead purposeful lives.

We have an exciting opportunity for an Outreach Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

You will focus on engaging with people who have a history of rough sleeping and substance misuse.

You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.

  • Hours: 37.5 per week
  • Contract: Permanent Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the Role

Going on outreach shifts; these include in all weathers on early mornings, nights, and weekends. Engaging with individuals experiencing rough sleeping and people involved in street-based activity. Working with partner agencies to support people using services to access accommodation, physical and mental health services, substance use services, benefits etc. Accurately recording interactions with individuals, you support and other professionals. About you: Experience of engaging and providing meaningful support to individuals with severe and multiple unmet needs. Experience of communicating effectively with people requiring support Experience of recording relevant sensitive information in a timely manner Knowledge of at least one of the following issues around homelessness; the benefits system, substance use, mental or physical health, the criminal justice system, housing (Social and/or Private Rented). Some experience of supporting people who may be affected by homelessness, mental health and substance use. Knowledge of Safeguarding Procedures and their application.
