Overview Physical Therapist, Home Health — Belle Fourche, South Dakota, United States.
Start date 06/22/2026.
13-week travel assignment in a long-term care setting with a guaranteed 40 hours per week.
Days shift.
Weekly pay range: $2,265–$2,487 per week.
This temporary assignment requires working in a nursing home environment while delivering PT services across resident care plans.

Responsibilities

Perform comprehensive physical therapy evaluations and establish individualized treatment plans for residents.
Implement PT interventions including therapeutic exercises, gait and balance training, range-of-motion work, transfer training, and functional mobility.
Monitor and document patient progress, adjust treatment plans, and communicate outcomes to the care team.
Provide patient and family education on exercises, safety, fall prevention, and home exercise programs; ensure continuity of care with interdisciplinary staff.
Maintain accurate, timely documentation in compliance with regulatory requirements and facility policies.
Ensure safety and infection control during all interventions and prioritize resident dignity and quality of life.

Requirements

Active Physical Therapist license in good standing in the United States.
Ability to work in a long-term care setting and collaborate with an interdisciplinary team.
Availability to start on 06/22/2026 and commit to a 13-week travel assignment.
Strong documentation and communication skills; ability to develop and update care plans and progress notes.
Adaptability to a days shift schedule and to working within a nursing home environment.
