Responsibilities

• Develop and implement a tailored plan of care for Home Health and Hospice patients, delivering skilled nursing services in patients’ homes
• Perform comprehensive assessments, manage symptoms, administer medications per orders, and address wound care as appropriate
• Maintain a high level of autonomy; work to the top of your license while coordinating with a multi-disciplinary care team
• Document all care activities and patient progress in the electronic medical record; update care plans accordingly
• Educate patients and families on disease management, safety, and care modalities; support caregivers to manage care at home
• Adapt to changes in patient needs and scheduling while maintaining quality and safety standards

Requirements

• Current active NH Nursing License and BLS certification
• Minimum 18 months Home Health experience
• Hospice experience preferred - 2-5 years Med-Surg experience preferred
• Self-directed and able to work without direct supervision
• Ability to adapt to home environments and diverse patient populations
• English: speak, write, read, and comprehend
• Computer proficiency in electronic medical records data entry
• Valid driver’s license and auto insurance
• All licenses/certs current at time of submission
• Territory: South Territory – Littleton/Plymouth areas
