Responsibilities

• Provide physical therapy services to LTAC patients under supervision of a licensed physical therapist
• Conduct patient assessments and contribute to the development and modification of treatment plans
• Implement therapeutic exercises, gait and balance training, transfer training, and mobility work
• Monitor patient response to therapy, document progress, and communicate with the care team
• Ensure safe patient handling, adherence to infection control practices, and proper equipment use

Requirements

• Active Physical Therapy Assistant license in New Hampshire or eligible for licensure
• Experience in LTAC or skilled nursing rehab settings preferred
• Strong communication and collaboration skills; ability to document clearly
• Ability to work the assigned shift and travel schedule for a 13-week assignment
