Responsibilities

• Provide physical therapy assistant services under supervising physical therapist in a LTAC/long-term care setting
• Conduct patient evaluations and document progress, including range of motion, strength, balance, and functional mobility
• Implement and progress treatment plans with goals such as gait training, therapeutic exercises, balance training, and activities of daily living (ADL) practice
• Administer modalities and therapeutic interventions as directed, ensuring patient safety and comfort
• Maintain accurate documentation of treatments, responses, and goals; communicate changes to the supervising PT and rehab team
• Ensure proper use of equipment, infection control, patient positioning, and adherence to safety protocols

Requirements

• Active Physical Therapy Assistant license in New Hampshire or eligibility for licensure
• Associate degree in Physical Therapy Assistant from an accredited program - 1 year of PT assistant experience in a rehab, LTAC, or skilled nursing setting preferred
• Ability to work effectively in a long-term care/assisted living environment
• Strong communication and documentation skills
