Coordinate care and connect clients experiencing behavioral health and substance use challenges with community resources. Develop individualized treatment plans and ensure accurate documentation in compliance with Kentucky Medicaid standards.
Requires a Bachelor's degree in Social Work, Psychology, Human Services, or a related field. Experience in behavioral health, substance abuse treatment, and knowledge of Kentucky Medicaid guidelines is preferred.
Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or related field required. Experience in behavioral health, substance abuse treatment, or case management preferred. Knowledge of Kentucky Medicaid Targeted Case Management guidelines preferred. Strong communication, organizational, and documentation skills. Ability to work independently and collaboratively within a multidisciplinary team. Basic computer and electronic health record (EHR) proficiency. Valid driver’s license and reliable transportation may be required. Preferred Skills Crisis management and de-escalation skills. Knowledge of community behavioral health resources in Kentucky. Experience working with individuals with co-occurring disorders. Cultural sensitivity and trauma-informed care approach. Ability to manage multiple cases effectively.
