Provide quality nursing care to patients in nursing centers and their homes under the direction of a Nurse Case Manager. Ensure all duties comply with State and Federal regulations and the Nurse Practice Act.
Requires an Oregon Licensed Practical Nurse certification and BLS from the American Heart Association within 30 days of hire. Must have at least 1 year of experience in acute care, home health, or skilled nursing facilities and a valid driver's license.
$3,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.
Area Coverage: Dallas, Northwest Salem, Corvallis and surrounding areas
The LPN provides quality care to patients in the nursing center and their homes under the direction of a Nurse Case Manager or Clinical Supervisor following the established nursing process and procedures, and in accordance with the Nurse Practice Act. Performs all duties in compliance with all applicable State and Federal regulations and in compliance with all regulatory bodies. LPN's working in Home Health require 25% to 35% of time on daily travel.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
