The Community Health Worker serves as a liaison to support vulnerable community members by addressing their material and health challenges. They maintain a caseload, conduct assessments, provide referrals, and support the implementation of care activities.
A high school diploma or equivalent is required, along with 1-3 years of relevant experience. The role involves mentoring and providing oversight to improve health outcomes for community members.
Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.
