Provide comprehensive community-based case management to homeless young adults to help them achieve housing stabilization and self-sufficiency. Responsibilities include conducting intake assessments, developing individual service plans, and liaising with community resources and landlords.
Requires an Associate's Degree and at least 2 years of relevant experience in direct service and case management. A valid Connecticut Driver's License and the ability to attend a mandatory orientation are also required.
