  • Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona.
  • CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

  • CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
  • CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.
  • Job Summary The Case Manager provides case management services in either the community and/or in office setting to patients and their families.
  • The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress.
  • The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural support.
  • The Case Manager assesses patient needs, develops and monitors individualized treatment plans and services provided in an accurate and timely manner.
  • Embraces the recovery philosophy and promotes a safe recovery environment for clients.
  • Skills/Requirements Highschool diploma or GED required.
  • Associates Degree or higher in a field related to behavioral health preferred.
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred.
  • 1-3 years of experience working with the GMH/SU population required.
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
  • 1-3 years of experience in a medical setting preferred Valid identification required Current AZ Driver's License (valid and in good standing), preferred.
  • 39-month MVR preferred Behavioral Health Technician (BHT) in accordance with A.A.C.
  • R9-10-101.33 and CBI Clinical Policies and Procedures Arizona Fingerprint Clearance card
