The Case Manager provides essential coordination, advocacy, and monitoring services for patients and families in community or office settings. They are responsible for assessing patient needs, developing individualized treatment plans, and connecting clients to health, financial, and social services.
A high school diploma or GED is required, with a preference for an associate degree in a behavioral health field. Candidates must have 1-3 years of experience working with GMH/SU populations and providing outreach and engagement activities.
