The Case Manager provides essential coordination and advocacy services to patients and families in community or office settings. They are responsible for assessing patient needs, developing individualized treatment plans, and connecting clients to health, financial, and social services.
A high school diploma or GED is required, while an associate degree in behavioral health is preferred. Candidates must have 1-3 years of experience working with general mental health and substance use populations, including outreach activities.
