Provide professional physical therapy evaluations and individualized treatment plans to help home health patients regain strength and independence. Collaborate with healthcare professionals and supervise Physical Therapist Assistants to ensure optimal patient outcomes.
Requires a current Massachusetts Physical Therapist License and a minimum of two years of experience as a Registered Physical Therapist. Home health or community health experience is preferred along with strong clinical assessment and communication skills.
$48.00 - $53.00 per hour, based on experience How You'll Make an Impact Provide professional physical therapy evaluations, treatments, and interventions in accordance with physician orders and the patient's plan of care. Assess patients' functional abilities, limitations, and rehabilitation potential through comprehensive evaluations. Develop and implement individualized treatment plans designed to restore function, maximize independence, and improve quality of life. Monitor patient progress and adjust treatment plans as appropriate to achieve optimal outcomes. Accurately document evaluations, treatments, patient responses, and changes in condition in accordance with agency policies and regulatory requirements. Educate patients, caregivers, and family members on treatment plans, safety recommendations, home exercise programs, and the use of assistive devices. Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care. Participate in the development and ongoing revision of patient care plans. Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) as appropriate. Coordinate patient equipment needs through approved medical supply vendors. Identify patient and family needs and facilitate referrals to community resources and support services. Participate in case conferences, community education initiatives, agency in-services, and continuing education opportunities to maintain clinical excellence.
