Intake Nurse, LPN - Home Health - Days (full-time)

General Summary

Provides intake and scheduling duties for VNA Home Care and Hospital at Home patients. Coordinates care delivery with appropriate Hospital at Home staff, Connected Care at Home staff and other ancillary services, as needed.
