Conduct initial assessments and provide physician-prescribed physical therapy to patients in their homes. Manage OASIS documentation and collaborate with healthcare personnel to develop and revise patient care plans.
Must be a licensed Physical Therapist in Texas and a graduate of an approved PT curriculum. Two years of experience and proficiency in OASIS visits are preferred.
Flexible schedule Summary: All About Home Care is a Houston-based home health care agency. We’ve been serving the Houston and surrounding communities since 2003, providing quality home health services. We are currently looking for a Physical Therapist PT to help serve patients in the Upper North coverage area of Greater Houston, including Spring, Conroe, Porter Heights, Tomball, The Woodlands, and surrounding areas. If you have experience in home health and are interested in covering these areas, please submit your resume. We use Kinnser software to document patient care.
Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association Currently licensed (licensed physical therapist) in the state of Texas. Two years experience preferred. Ability to exercise initiative and independent judgment. Spanish speaking a plus Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)
Makes the initial assessment visit and generates OASIS documentation the same day of the visit Understands and adheres to establish policies and procedures. Provides physician prescribed physical therapy. Performs evaluation and treatment visits. Improves or minimize the residual physical disabilities of the patient. Returns the individual to optimal and productive level within the patient’s capabilities. Participates with all other healthcare personnel in patient care planning. Direct and supervise their personnel as required. Performs all skilled procedures as ordered by physician. Consults with physicians regarding change in treatment ,reports to physicians regarding patients progress, instructs patients and families/significant others in home programs and activities of daily living. Participates in in-service programs and presents in service programs as assigned. Participates in performance improvement activities as assigned. Attends all patient care conferences as scheduled. Prepares clinical and progress notes daily. Assists physicians with evaluating level of function. Helps develop the plan of care and revise as necessary. Consults with family and agency personnel.
