Physical Therapist (PT) – Home Health

As a Home Health Physical Therapist, you’ll practice at the top of your license while delivering personalized, one-on-one care in the comfort of patients’ homes.

As a physical therapist, you will

• Perform and document comprehensive initial evaluations to identify patient needs and establish individualized treatment plans.
• Assess physical conditions and determine functional goals prior to intervention.
• Develop and implement customized physical therapy programs to improve or restore mobility and physical function.
• Evaluate treatment effectiveness and modify care plans to maximize patient outcomes.
• Educate patients and caregivers on treatment plans, exercises, and safety techniques.
• Apply infection control and universal precautions while delivering patient care.
• Ensure continuity of quality patient care through timely and accurate documentation.
• Communicate effectively with patients, families, interdisciplinary teams, and healthcare professionals.
• Support agency philosophy, mission, and clinical standards.
• Complete documentation within company-established guidelines.

Requirements

• Graduate of an accredited Physical Therapy program
• Current and unrestricted Physical Therapist license
• Minimum 1 year of Home Health experience preferred, not required
• Current CPR certification
• Valid driver’s license and reliable transportation
• Current auto insurance
• HomeCare HomeBase experience preferred

• Physical demands
• Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods
• Ability to lift, carry, push, or pull up to 100 lbs.
