Hospice Care, Bereavement Counselor (LCSW or LMSW)

Job Description

Provides counseling services to bereaved family members of hospice patients and the general community.

Job Responsibility

• 1.Assesses bereaved clients and provides program orientation.
• 2.Collaborates with healthcare team members to develop and implement client plan of care.
• 3.Conducts individual, group and family counseling sessions.
• 4.Serves as a liaison to referral sources to facilitate continuity of care.
• 5.Performs related duties as required.

• Job Qualification
• Master's Degree required.
• Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State, or Current License to practice as a Mental Health Counselor in New York State required, plus specialized certifications as needed.
• 1-3 years of technical experience required.
• Minimum of 1 year of bereavement and grief counseling experience strongly preferred.
• Experience with individual and group sessions and assessment, experience working with all ages, and families preferred.
• Hospice care, home care, or community health experience preferred.
