What You’ll Do
Provide skilled nursing visits in patient homes and care settings.
Deliver treatments including wound care, medication management, and assessments.
Coordinate care with physicians, therapists, and interdisciplinary teams.
Educate patients and families on care plans, medications, and recovery goals.
Maintain accurate, timely documentation in the EMR.
What We’re Looking For
Active Minnesota Registered Nurse (RN) license.
Experience in home health, med-surg, TCU, rehab, or clinical nursing.
Strong clinical judgment and ability to work independently.
Excellent communication and patient education skills.
Valid driver’s license and reliable transportation.
