Develop individualized treatment plans focused on meaningful functional outcomes. Provide evidence-based interventions for communication, cognition, and dysphagia. Educate patients, caregivers, and family members to promote long-term success. Collaborate closely with physicians, nurses, therapists, and the interdisciplinary care team. Complete timely and accurate documentation using our electronic medical record. What We're Looking For Active Minnesota Speech-Language Pathologist (SLP) license. Passion for helping patients regain independence and confidence. Excellent communication and clinical reasoning skills. Home health experience is welcomed but not required.
