What You'll Do Carry out individualized treatment plans developed by the supervising Physical Therapist. Provide therapeutic exercises, gait training, transfer training, balance activities, and functional mobility interventions. Reinforce home exercise programs and educate patients and caregivers. Monitor patient progress and communicate updates with the supervising PT. Collaborate with physicians, nurses, and the interdisciplinary care team. Complete accurate and timely clinical documentation. What We're Looking For Active Minnesota Licensed Physical Therapist Assistant (PTA) license. Passion for helping patients regain independence. Strong communication, organization, and patient education skills. Home health experience is preferred but not required. Valid driver's license and reliable transportation. Requirements

Must hold an active Minnesota Licensed Physical Therapist Assistant (PTA) license and a valid driver's license with reliable transportation. Home health experience is preferred but not required.
