What You'll Be Doing Provide skilled RN home health visits throughout the Twin Cities service area Complete comprehensive nursing assessments Perform wound care, medication management, injections, and other skilled nursing services Educate patients and caregivers on disease management and recovery Collaborate with physicians, therapists, and interdisciplinary care teams Support multiple territories based on patient needs and census Complete accurate documentation using Homecare Homebase (HCHB) Help ensure patients receive timely, uninterrupted care Who We're Looking For Active Minnesota Registered Nurse (RN) license Experience in home health, hospital, rehabilitation, or skilled nursing preferred Strong assessment and critical thinking skills Comfortable working independently in a field-based environment Flexible and adaptable to changing daily assignments Reliable transportation and willingness to travel throughout the Twin Cities Metro About Home Health Care, Inc.
