What You'll Do Provide skilled nursing visits in patients' homes. Perform wound care, medication management, and other skilled nursing interventions. Educate patients and family members on treatments, medications, and disease management. Collaborate closely with RN Case Managers, physicians, therapists, and the interdisciplinary care team. Document visits accurately and ensure continuity of care. What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license. Ability to work independently while providing compassionate, patient-centered care. Reliable vehicle, valid driver's license, and current auto insurance. Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred. EMR experience is a plus, but we'll help you learn our systems.
