A collaborative team that values your expertise and supports your success. What You'll Do Evaluate and treat patients throughout Southern and Southwest Minnesota in their homes. Design individualized treatment plans focused on restoring mobility and independence. Educate patients and caregivers to improve long-term outcomes. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the care team. Monitor progress and adjust treatment plans as patients improve. Complete timely and accurate documentation. What We're Looking For Active Minnesota Physical Therapist (PT) license. Strong clinical reasoning and patient-centered approach. Excellent communication and organizational skills. Comfortable working independently across a regional territory. Valid driver's license and reliable transportation. About Home Health Care, Inc.
